Medical Insurance

Accidental Death and Dismemberment Insurance

Short Term Disability Insurance

  • Lincoln Financial. Company pays for employee
  • Coverage begins the 1st day of an accident, 8th day of an illness/maternity leave, 1st day of hospitalization
  • Pays 60% of weekly salary up to $750.00

Long Term Disability Insurance

  • Lincoln Financial. Voluntary benefit, after-tax payroll deduction
  • After-tax payroll deduction, pays 60% of monthly salary up to $5,000.00
